1. About the service
FSI Health Card ("FSI", "we") provides a digital healthcare membership offering access to a network of partner hospitals and doctors, appointment booking, medical report storage, a member wallet and a referral programme. FSI is not an insurance product and does not provide medical advice or treatment.
2. Membership
Membership is valid for the period stated on your plan from the date of activation and must be renewed to continue. You must provide accurate information and keep your password and transaction PIN confidential. One account per person.
3. Healthcare providers
Medical services are provided solely by independent hospitals and doctors. FSI verifies listings but is not responsible for clinical decisions, treatment outcomes, fees or availability at any provider.
4. Wallet
Wallet balances arise from commissions and member transfers and can be used only within the FSI platform as permitted. Transfers require your transaction PIN and are final once successful. FSI may hold, reverse or adjust transactions in case of error, refund or suspected fraud.
5. Referral programme & rewards
Commissions are paid only on successfully paid and activated cards at the rates published at the time of sale, and are reversed if the underlying payment is refunded. Rewards are subject to verification, eligibility rules and availability. Income is not guaranteed and depends entirely on genuine card sales. Misrepresentation, fake accounts or manipulation will lead to forfeiture and account suspension.
6. Suspension
FSI may suspend or block any account for misuse, fraud or breach of these terms. Renewal of a membership does not lift a security suspension.
7. Liability
To the extent permitted by law, FSI's liability is limited to the membership fee paid for the current period.
8. Governing law
These terms are governed by the laws of India.
